If you need to submit a fingerprint card for a background check to the Florida Board of Nursing, especially if you're an out-of-state applicant or unable to visit a Live Scan provider, follow these steps:

1. Use an Approved Live Scan Provider

  • The Florida Board of Nursing generally requires fingerprints to be submitted electronically via Live Scan fingerprinting in Miami or other locations.

  • If you're out of state or cannot access a Live Scan provider, you may submit your fingerprints using an FD-258 fingerprint card. Contact a certified fingerprint service provider that can convert your fingerprint card into an electronic submission compatible with the Florida Department of Law Enforcement (FDLE).

2. Complete the Fingerprint Card

  • Have your fingerprints taken on an FD-258 card by a law enforcement agency or certified fingerprinting near you.

  • Ensure the fingerprint card is properly completed with accurate personal information (e.g., name, date of birth, and other required details).

3. Submit the Fingerprint Card to a Third-Party Processor

  • Fingerprint cards cannot be sent directly to the Florida Board of Nursing or FDLE. Instead, mail the card to an approved third-party processor who will scan and electronically submit your fingerprints to the FDLE.

  • We provide services for converting fingerprint cards in Miami into electronic submissions. Check with the Florida Board of Nursing for additional approved vendors if necessary.

4. Include Required Information

  • When submitting your fingerprint card through a third-party processor, make sure to include:

    • The ORI number specific to the Florida Board of Nursing. This ensures your background check results are sent to the correct agency.

    • Payment for processing, as required by both the vendor and the FDLE.

5. Track Your Submission

  • After your fingerprints are electronically submitted, you will receive a Transaction Control Number (TCN). Use this number to track the progress of your background check through the FDLE.

Summary of Steps:

  1. Have your fingerprints taken on an FD-258 card by a certified provider.

  2. Send the completed card to an approved third-party processor who will submit your fingerprints electronically to FDLE.

  3. Provide the required ORI number and payment.

  4. Track your submission using the TCN provided.
