Miami Dade Apostille helps prepare your request and works with a channeler to process eligible FBI Identity History Summary requests. We are not the FBI or any government agency. Use this form to start your request — our office will review it and contact you with next steps.

Channeler eligibility

FBI channeling is generally available to U.S. citizens and lawful permanent residents located in the United States. If you are outside the U.S. or are not a U.S. citizen or LPR, please call us or email us before submitting, or submit directly through the FBI directly . International kit shipping is a separate question from channeler eligibility.

Mail-In

Mail two original FD-1164 card sets with tracking to our Miami office. We review your cards, invoice you, and submit to the channeler after payment and eligibility confirmation.

Fingerprint Card Kit

We ship you a kit. You get inked locally (police station, notary, or fingerprint technician) or follow the included instructions yourself, then mail the completed cards back to us. Unclear prints may require a retake.

Channeling service is NOT included in the kit price.

Kit price: $50 U.S. shipping / $99 outside the U.S. — paid before kit ships. Channeling fees are quoted and invoiced separately after we receive your completed cards.
