Lake Alfred vital-record office
The local Office of Vital Statistics is in the Northeast Government Center, 200 Government Center Boulevard, Lake Alfred, FL 33850. Call 863-519-8446. Published hours are Monday–Friday, 8 a.m.–4:30 p.m. This office replaced the former Bartow vital-record counter in 2023. Confirm the current service and your record's eligibility before visiting; an older application may show a different physical office.
Vital-record inquiries in Lakeland
The county's birth and death certificate guidance also identifies its Lakeland location at 3241 Lakeland Hills Boulevard, Lakeland, FL 33805. The clinic's general number is 863-519-7910; the county vital-record line is 863-519-8446. Ask specifically about certificate service and counter hours. Clinical opening hours and same-day medical appointments are not a promise of certificate pickup.
Confirm pickup before making the trip
Ask whether the custodian can issue your particular certificate during your visit and whether same-day pickup is available. The current county guidance does not guarantee same-day completion for every request. Identification checks, older entries, sealed records or corrections can change the schedule. Birth certificates: year, identity and issuing state
Polk's guidance lists local Florida birth certificates from 1930 onward and directs earlier or sealed records to the Bureau of Vital Statistics. Applicants need the custodian's required identification and the names, birth date and other details needed to find the entry. A birth outside Florida comes from its own issuing jurisdiction. Confirm the recipient's requested version with our team before ordering for international use.
Death records and the requested version
The county lists local Florida death certificates from 2009 onward; older records follow the state vital-record or VitalChek route. Access to cause-of-death information is restricted. Tell the custodian which version is needed and confirm eligibility and identification requirements. Miami Dade Apostille reviews the copy for the agreed service, while the receiving institution decides what its application requires.

VitalChek is the third-party online ordering service authorized by Florida's health department. It offers an alternative to an in-person vital-record request. The Clerk lists Polk County Courthouse at 255 N. Broadway Avenue, Bartow, FL 33830; Lakeland Government Center at 930 E. Parker Street, Lakeland, FL 33801; and Northeast Government Center at 200 Government Center Boulevard, Lake Alfred, FL 33850. Call 863-534-4000 to confirm the correct division and service before visiting. Published Clerk hours are weekdays, 8 a.m.–4:30 p.m., excluding holidays. The Clerk and vital-record office have separate functions even at the same government center.
Marriage records after the license is recorded
For a Polk marriage record, request the certified completed record from the Clerk. The ceremony and recording of the returned license come before obtaining the completed certified marriage record. An ordinary online image or a decorative certificate is not automatically the version needed abroad. Divorce judgments and other court documents
A divorce certificate and the court's final divorce judgment are different records. If the recipient requests the decree or another court order, ask the Clerk's records staff for that specific certified document using the case number and party names. Probate, family and other restricted case files may need access review. The Clerk lists [email protected] for records inquiries; confirm any retrieval and physical-copy arrangements before traveling.
Electronic certification has a specific local limit
Polk's current records guidance offers electronic certification for Official Records, including eligible recorded marriage licenses and deeds, but states that the service is not currently available for court records. Other certified copies can be requested for processing and delivery. Before purchasing an electronic certificate, contact Miami Dade Apostille to review the needed format. A digital file or printed copy is not universally accepted by every authority or foreign recipient.
Finding historical deeds and recording references
The Clerk's online Official Records collection covers 1957 onward, and its separate historical-deed collection covers 1861–1956. Names, dates and recording references help the custodian identify the right document. A search entry or freely downloaded image is not itself a certified copy. Ask about older or restricted material rather than assuming every record is immediately available online.
Polk State College records assistance nearby
Polk State identifies full Registrar services in Winter Haven, Lakeland and at the JD Alexander Center in Lake Wales. Polk State uses Parchment for official transcript requests and describes secure PDF and FASTER electronic delivery. An unofficial portal printout is different from an official transcript. For a document intended for apostille or additional certification, contact Miami Dade Apostille and the Registrar about the required format before ordering. Electronic delivery speed does not establish immediate campus pickup or completion of any notarization, apostille, translation or shipping.
School records come from their own custodian
For a high-school transcript, contact the school or the district office responsible for its archived records. For another college, contact that institution. Polk State cannot issue another institution's transcript, and a GED record has its own issuing authority. Contact Miami Dade Apostille before signing a document that may need notarization or witnesses. Remote Online Notarization is available for eligible documents, subject to identity checks and recipient requirements. A notarized photocopy does not replace a birth, marriage or court record that must be certified by its proper custodian.
