For birth and death records, the Lake County Administration / Vitals office is at 16140 US Hwy 441, Eustis, FL 32726. Call 352-589-6424. Its location page lists Monday–Friday hours of 8 a.m.–4:30 p.m. Confirm availability, eligibility and the counter cutoff before traveling. This is an office for obtaining the underlying record, not the delivery address for your apostille order.

Clermont vital-record office

The Clermont Health Center provides vital-record services at 875 Oakley Seaver Drive, Clermont, FL 34711; call 352-771-5500. Vital Statistics lists Monday–Friday hours of 8 a.m.–4:30 p.m., with a noon–1 p.m. closure. General building hours are longer, so plan around the certificate counter's hours. Contact the office before a visit to confirm your record and required identification.

Leesburg vital-record office

The Leesburg Health Center is at 2113 W. Griffin Road, Leesburg, FL 34748; call 352-771-5500. Vital Statistics lists Monday–Friday service from 8 a.m.–4:30 p.m. and is unavailable from noon–1 p.m. The general health-center closing time is different. Confirm the certificate service and documents you need before traveling, then contact our team about apostille preparation.

Local pickup can be quick, but is a separate service

Lake's birth and death certificate pages say an eligible in-person request usually takes approximately 15 minutes. That supports the possibility of same-day record pickup when the record is available and the application, identification and eligibility requirements are met. It is not a guarantee for every record or visit. Ask the chosen office to confirm current availability. Once the certificate is obtained, Miami Dade Apostille reviews it and coordinates the separate apostille or authentication stage.

Florida birth and death record requirements

Lake's birth-certificate page describes Florida births from 1930 onward and requires an eligible requester with identification. Ask about older or restricted records before visiting. Lake's certificate overview lists Florida deaths from 2009 onward; ask the office about earlier records. Cause-of-death information requires appropriate authorization. Tavares courthouse: marriage, court and recorded copies

The Lake County Courthouse is at 550 W. Main Street, Tavares, FL 32778; general hours are Monday–Friday, 8:30 a.m.–5 p.m. Marriage and Recording are in the North Wing, third floor. Court Records and Domestic Relations are in the East Wing, first floor. Call 352-253-2648 for Marriage, 352-253-2600 for Recording, 352-742-4336 for Court Records or 352-742-4140 for Divorce/Domestic Relations. Confirm the particular copy and pickup arrangements with the relevant department.

Marriage copies at the North Lake and South Lake branches

The Clerk says certified marriage copies can be purchased at all three marriage-license locations. North Lake is at 902 Avenida Central, The Villages of Lady Lake; call 352-253-4575. South Lake is at 290 Citrus Tower Boulevard, Suite 116, Clermont, FL 34711; call 352-253-4589. Both list Monday–Friday hours of 8:30 a.m.–4:30 p.m., closed noon–1 p.m. Confirm service before traveling. Do not assume every court-record service is available at these branches.

Official Records and the Tavares storage facility

For certified recorded documents, the Clerk also lists an in-person option at the Records Storage Facility, 313 S. Bloxham Avenue, Tavares, FL 32778. General hours are Monday–Friday, 8:30 a.m.–5 p.m.; call Recording at 352-253-2600. Deeds and other Official Records are a different collection from court case files. The physical and postal destinations listed by a record office are not Miami Dade Apostille's delivery instructions.

Electronic certified records need a format check

Lake offers eCertified Official Records with a digital Clerk signature. Only records available for viewing on its website can be purchased through that online service. The Clerk advises confirming that the recipient accepts electronic certification before purchase; purchases are nonrefundable. A manually certified paper copy is another option when available. Contact Miami Dade Apostille before choosing a format. Keep the original electronic file if one is issued; a printout is not the same as the signed digital record.

Marriage records and divorce judgments are different requests

A certified marriage record comes from the issuing Clerk after the completed license has been returned and recorded. A certified final divorce judgment comes from the Clerk handling that court case. It is different from a state dissolution certificate, which summarizes the event. For Lake cases, contact the appropriate courthouse department with the record or case details. Affidavits, powers of attorney and notarized documents

Privately signed documents may need appropriate notarization, witnesses or other preparation before certification. Florida notarized records require a complete English notarial certificate. For a diploma or transcript, contact the actual school or registrar about the record, certification and pickup location. There is no single county office for every academic document.
